The Philippine Charity Sweepstakes Office (PCSO) has established an Individual Medical Assistance Program (IMAP) to aid the financial needs of Filipinos with health-related problems.
Once approved, PCSO will issue a guarantee letter to the hospital or partner health facility, assuming the obligations of a specific amount due to the eligible client for the services rendered.
All Filipino patients who request assistance, either personally or through a representative, on a health-related concern, including:
Eligible patients who are currently admitted or have been discharged with promissory notes duly acknowledged by the health facility.
Eligible patients who are undergoing, such as, but not limited to Hemodialysis and Peritoneal (Continuous Ambulatory Peritoneal Dialysis or CAPD).
Cancer patients who are undergoing therapies, such as, but not limited to:

Listed below are the necessary documentary requirements you will need to apply for the PCSO IMAP Medical Assistance:
Duly accomplished PCSO IMAP application form (Click here to download the application form)
Original or Certified True Copy of the Clinical Abstract duly signed by the attending physician with license number
Valid ID of patient and representative
Authorization letter from the patient in the case where there is no immediate relative available for interview
For each different type of treatment, the applicant will need to bring additional requirements to prove you are indeed suffering from an illness and taking the appropriate treatment. See this link for more details on the requirements.
Ask for a Clinical Abstract from the doctor or the doctors in charge of the patient;
Ask for the Official Hospital Billing Statement from the hospital officials, especially from the finance department. Explain the need to have this for PCSO purposes;
Ask for Certification from the Barangay Captain, noting that you are a relative of the patient and you are incapable of paying for your hospital bills;
Make a Letter addressed to the PSCO manager or the chairman stating the condition of your patient and your need for assistance. See the needed details below:

Email the documents to ncrmap@pcso.gov.ph. Write the subject line with the name of the patient and the type of assistance request applied for. Wait to receive a notification of the status of your request within 24 hours.
Once you receive the notification, you can submit the printed Guarantee Letter, MAP Assessment Form, and MAP Application form, including the complete documentary requirements, to the Hospital, Dialysis Center, Partner Health Facility (PHF)/medicine retailer, or Diagnostic Center.
As of July 2020, PSCO has released over P153 million in medical assistance to a total of 21,240 indigent beneficiaries.
In a statement released on August 5, PCSO general manager Royina Garma said of the total amount of assistance, P74.4 million was given to 8,378 patients that needed hospital confinement, P54.1 million went to dialysis sessions of 10,675 patients, and P22.9 million was given to 1,957 patients needing chemotherapy treatments while post-operation medication amounting to P2.1 million was given to 213 patients and assistance for hemophilia treatments amounting to P158,100.00 was provided to 17 individuals.
The NCR Department released the highest amount of assistance at P39 million for medical expenses of 3,536 patients. This was followed by the Northern and Central Luzon (NCL) Department providing P38 million to 5,400 patients; the Southern Tagalog and Bicol Region (STBR) released P28.9 million as a provision for 4,943 patients; the Visayas Department granted P26.5 million to 3,901 patients, and; Mindanao Department dispensed P21.2 million to 3,460 patients.
